The Emergence of ECMO (Extracorporeal Membrane Oxygenation) Into EMS (#E0091) (Room 208B)

Extracorporeal Membrane Oxygenation (ECMO) is not a new technology, but it's used as rescue therapy for failed attempts at resuscitation of out-of-hospital cardiac arrest (OHCA) victims is building in communities worldwide. EMS and hospital-based ECMO programs must work together closely to ensure the best outcomes for these patients. Dr. Youngquist will discuss the development of an ECMO pathway for OHCA victims in Salt Lake City, including challenges of patient selection, determining appropriate time on scene, and the use of mechanical CPR devices to reduce interruptions in CPR during transport.

Paris SAMU physician, Dr. Lionel Lamhaut, will introduce you to one of the world’s first prehospital ECMO response systems that responds along with other BLS and ALS resources to difficult-to-resuscitate cardiac arrest cases in Paris and has an amazing 38% resuscitation rate for these nearly dead patients. He will detail the selection criteria utilized for ECMO response, the type of equipment deployed and the on-scene choreographed care for this lifesaving procedure. Dr. Lamhaut will also discuss what he sees as the future for ECMO performance in the prehospital setting.

Michael Jacobs will briefly discuss a case where a STEMI patient treated and transported in Alameda County coded in the hospital and was resuscitated successfully after close to 6 hours on a LUCAS device and 7 days on in-hospital ECMO.
