(PP311) Continuity Through the Continuum (Room 510)

A panel will present their challenges and solutions for caring for a new ostomy patient across the healthcare continuum.The current healthcare climate in North America is one in which healthcare professionals are challenged to care for individuals as they rapidly move between the various healthcare settings receiving various levels of care. Prolonged acute care stays are limited and the acuity of individuals in the homecare and long-term care settings is rising. Wound Ostomy and Continence Nurses are challenged to ensure continuity of care among the healthcare settings. Budgetary restrictions in different sectors often results in a disruption in access to care, dressings and specialists. This session will explore the challenges and barriers to providing best practice care in these challenging times.
