OneTouch Journal provides consumers with a mobile application to actively particpate in their care. Using a patient's data, our data-driven technology auto-generates healthy activities for the patient to complete and care plans. A patient may also chat with clinical staff online. Video education is populated based on the patient's diagnoses. Our newest release allows a patient to consolidate their EHR Continuity of Care (CCD) record by using direct messaging to assemble all their records in one secure repository and then share with their providers.
As a Medicare qualified registry we report provider quality, advancing care information (ACI) and improvement activities using our HealthE QM technology. Providers receive electronic performance scorecards each month.
