Tim is the Chief Operating Officer (COO) at Fazzi Associates. A leader in the field of home care and hospice, Tim’s expertise and career has included a unique blend of clinical, operational, fiscal and academic roles. Tim is a frequent presenter at national and state conferences and is often asked to contribute to industry forums. Tim provides organizational, operational, turn around, and change management services to home care agencies across the country and is highly skilled at helping agencies to create real change and progress on their path to Triple Aim outcomes. He is also the Co-Director of the 2016-2017 National Home Care and Hospice State of the Industry Study. Tim joined Fazzi in 2006, became a Partner in 2007 and was promoted to COO in 2017. Prior to joining Fazzi, Tim was the Vice President of Clinical Operations for Visiting Nurse Service of Rochester & Monroe County in New York. He was also an Assistant Professor at the University of Rochester School of Nursing. He earned his MBA and MS from the University of Rochester and his BSN from St. John Fisher College.
