Village Family Practice


Dr. Clive Fields is president of Village Family Practice, a Houston-based accountable care organization that is recognized for its quality of care by the Centers for Medicare and Medicaid Services (CMS) and the National Committee for Quality Assurance (NCQA). Board certified in Family Medicine, Dr. Fields has led Village’s team of more than 40 clinicians since 1991. A pioneer in the delivery of value-based care, Dr. Fields spearheaded Village’s participation in the 1990’s Medicare Plus Choice program, a precursor to value-based programs. In 2013, Dr. Fields parlayed his expertise as both a clinician and physician executive to co-found VillageMD, a premier primary care management services provider, where he also serves as the company’s chief medical officer. VillageMD partners with physician-led practices across the U.S. to help them transition to a value-driven model of care. In addition to his clinical practice and leadership role at VillageMD, Dr. Fields is an Associate Professor of Family Medicine at Baylor College of Medicine and University of Texas at Houston. He is on staff at Hermann/Memorial City Hospital, The Methodist Hospital, and St. Luke’s Hospital. Dr. Fields received his medical degree from the Chicago Medical School and a bachelor’s degree from the University of Texas at Austin. He is a member of the American Academy of Family Physicians. Dr. Fields is consistently named one of Houston’s “Top Doctors” by H Magazine and Texas Monthly, and is a regular contributor on health care trends to publications such as Forbes, Health Data Management and the Houston Chronicle.
