In April 2015, Complete Wind Corporation (CWC) experienced a fire in a wind turbine rotor blade while completing a repair. This incident resulted in CWC’s biggest failure to a Customer and as a Corporation. Word of the incident (and variations of the original occurrence) quickly spread throughout the tight-knit Wind Community, and to other Customers of CWC. During the Incident Investigation, various policy and procedural infractions by CWC employees were discovered, in addition to onsite violation of an objective and conclusive investigation by the Customer. Initially, we will review the incident itself and employee safety, consequences of the incident (i.e. loss of work to Corporation, lay-offs/terminations, etc.), results of the root cause analysis and lessons learned. We will then delve into the resulting CWC Organizational changes made in an effort to ensure not only Safe Work Practices, but the Quality of work CWC completes for Customers; these changes range from Recruitment and Selection of Appropriate Personnel, Training and Supervision as well as Procedural Modifications inclusive of Project Specific Training. These elements of change will be discussed in detail and an examination of our safety record as a result of these changes will be outlined.
