Attendees to this session will learn how daily patient engagement can improve care of defined health conditions. The speakers will present a case study of an organization that used an innovative digital health platform to foster daily interactions with over 500 patients with type 2 diabetes, CHF, COPD, and obesity. The presentation will then review the empirical findings of this initiative and demonstrate how engagement can be the leading mitigation factor for risk and total cost of care. Discussion of specific issues will be generated through audience polling.
