Improving America’s health system requires taking better care of the patients with the costliest and most complex conditions. These individuals, who account for 5 percent of the U.S. population but 50 percent of healthcare spending, typically have multiple conditions that are exacerbated by challenging social needs and a limited ability to care for themselves. Higher-quality, more-affordable care for high-need, high-cost (HNHC) patients requires collaboration among healthcare providers, payers, communities, social service organizations, academics, researchers, and others. In this session, you’ll explore strategies from the Commonwealth Fund, a participant in a six foundation collaborative that is seeking to crack the code to improve efficiency and effectiveness of care at a lower cost to high-need patients.
