A misdirected incident investigation doesn't correct safety problems - it merely allows them to hide. This only increases the likelihood of repetitive incidents, human hardship and financial loss. This seminar teaches a systematic approach to incident investigation to properly identify causal factors so appropriate corrective actions can be implemented.
