Brad Livingston was involved in back-to-back explosions that were 100 percent preventable. The contributing factors were those that exist in every organization: shortcuts, complacency, pride, bad attitudes and improper perspectives. Brad overcame a 5 percent chance of surviving his injuries and now shares his story. He explains the “ripple effect,” including what he – and, more importantly, his family – went through to drive home the message that these factors cannot be part of the workplace. “You think it can’t happen to you?”
