Although the concept of skin health is central to wound, ostomy and continence nursing, we continue to create silos around the various forms of skin damage based on cause and treatment. The purpose of this presentation is to highlight shared factors that threaten skin health in patients with acute, chronic and critical illnesses and provide a vision for a unified approach to the prevention, assessment and management of skin health that address the most common factors (pressure, shear, friction, trauma and moisture) that threaten our patient’s largest organ, the skin.
