Safety management systems often are built on a foundation of continuous improvement. However, analysis of accidents shows that organizations can continually improve yet still have tragic outcomes. Often, this is because organizations experience changes, but their management systems are ill-equipped to identify and deal with these changes. Professor Andrew Hopkins called this a “failure to learn.” In this session, we’ll review another aspect of achieving safety excellence – continuous learning. You’ll hear about relevant research and case studies, and participate in an interactive discussion to discover why organizational learning is a crucial element of safety excellence.
