Crane safety incidents are infrequent but carry a large potential for loss of life and property damage. The leading cause of fatalities in crane accidents is being struck by the load, most commonly due to rigging failures. Many factors can lead to rigging failures, but a common cause is cumulative fatigue. Over the last five years, three near-miss incidents involving crane hooks that failed by fatigue were investigated by SSAB Americas R&D, which highlighted the need for more stringent inspections. In this paper, the results of these failure analyses will be presented to raise awareness of the dangers of fatigue.
