The cost of poor care transitions of Medicare patients from acute care alone is $26 billion per year as of 2016. This session will describe the American Case Management Association’s (ACMA) Transitions of Care standards, recently released and developed by convening national healthcare leaders representing all health care delivery segments. These standards seek to bring clarity to the practice of care transitions and assist providers, payers and all health care organizations in establishing processes for seamless coordination across the entire continuum of care, with the goal of achieving, the best health outcomes.
