Home care and hospice organizations have great potential for fostering more affordable, high-quality care in accountable care organizations (ACOs). But what will it take for post-acute providers to effectively capture and demonstrate their value in the global quest to better manage the health of communities and designated populations? In this session, we will explore ways to develop collaborative relationships that build and sustain ACOs, while improving internal processes and procedures for excelling in a value-based health care environment.
